Both wholesale and retail customers may mail, fax or email this form only if you are not using the online shopping cart. If you are a retail customer, fill in all the information and submit the form as per the instructions below. We will add the applicable tax. If you are a wholesale customer, enter the retail prices in the boxes, following any retail ordering instructions, and add them to get the sub-total. Apply the wholesale discount as per the instructions that were sent to you in response to your Wholesale Inquiry. Enter that number in the box for the final total. (If you prefer to use the shopping cart online instead of this form, once you are a verified wholesale customer and you have created an account online, the wholesale discount will be applied automatically when you confirm your order.) The minimum amount for wholesale orders is $200. Please review our payment, shipping and returns policies.

Selected jewelry contains options with respect to the metal (sterling silver and 14k gold-filled). When ordering with the order form, if the jewelry is pictured in sterling silver, but you would like to order the jewelry in 14k gold-filled, add $4.00 to the listed retail price. If the item is pictured in 14k gold-filled and you would like to order it in sterling silver, subtract $4.00 from the listed retail price. The listed retail price is the same for all of the color options. State your selected option in the "notes" section of the order form.

Mail: To place an order by mail, print this form and fill in your selections. To pay by money order, you may mail a money order made payable to Pam Branch Designs and your completed order form to Pam Branch Designs, 2838 Galahad Way SW, Marietta, GA 30064. You may also pay by credit card by mailing this completed form, which must include your credit card information.
Fax: To place an order by fax, print this form, fill in your selections and your credit card information, then fax the form to (678)401-4729.
